This was more tied to the episode but I think if I was going to do a full AU of that question - I'd look at what could happen if Sam had told someone before the Demetrius mission, (though I guess that would have to presume Kara didn't joke about shooting him-- or if he'd confessed before she had the chance to say it). It's an interesting thing to think about, that's for sure!
